An employee was performing a test on a diesel fuel tank that is 10 feet high. He used an extension ladder to reach the top of the tank. While descending the ladder, he fell 9 feet to the ground and struck his head, suffering a concussion and skull fracture. The ladder was not tied off at the time of the incident.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

Absolics Inc.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

American Residential Products, Inc.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

Berkheimer Maintenance

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

High-Tec Industrial Services, Inc.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

Two employees were working on a forklift with its mast in the air when a chain came loose and the carriage fell down on them. The injured employee was hospitalized with a fractured left ankle, a laceration on the left side of their forehead, and a fractured L1 vertebra.

An employee was operating a machine used to roll erosion control fabric. Her left hand was caught and crushed in the rollers. She was hospitalized.

An employee was using a pipe grooving machine to groove an 8-inch pipe. His gloved index finger on his left hand was caught between the groover and the pipe, crushing the top part of his finger. The employee's left index finger was partially amputated.

An employee was cutting electrical wires when an arc flash occurred, resulting in third-degree burns to their hand.

An employee was being trained on operating a powered industrial truck (PIT) with a side-saddle stance. The employee was instructed to move the PIT s joystick back and forward to get a feel for it. The PIT went backward through a dock door that was approximately 3 feet off the ground. The PIT ended up on the employee. The employee was hospitalized with an amputation below the left knee, third-degree burns to lower extremities, and multiple fractures to the lower extremities.

Howard Industries Inc.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

Hobby Lobby Stores, Inc.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

Owl's Head Alloys West Point

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

Ingalls Shipbuilding

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
