An employee fell 14 feet from a fixed ladder while climbing up to access the roof, hitting the ground below. He suffered two fractured ankles, a fractured pelvis, and other bruises and contusions.

A ladder to the roof was set up on the loading dock. An employee was on the loading dock and getting ready to climb the ladder when the employee fell 3.5 feet off the loading dock. The employee was hospitalized with a fractured skull and broken collar bone.
The employee was on top of a rail car to level the grain that was deposited into the car when they fell 13-15 feet to the ground. The employee sustained a right leg fracture.
