An employee was using a hoist and sling to move a shaft. After the employee hooked the sling to the shaft, the up button was pressed on the hoist's wireless remote. The strap then tightened on the shaft and the employee's right hand was caught between the two, causing an amputation of the right middle fingertip above the knuckle.
