An employee was closing the front gate when it came off the track and fell on top of the employee. The employee was hospitalized with a fractured pelvis, compression fractures to the spine and a right rib fracture.

The pallet began to roll off the rack and pinned the employee against the transporter. He suffered a broken right leg.
