An employee and two coworkers were attempting to unload hay that was strapped on the back of a truck. When he unstrapped the hay elevator, it slipped and fell 4 feet. The motor portion of the elevator struck his head and mouth, causing a severe laceration to his scalp and a loss of teeth.
