A flaker operator was trying to unplug a flaker's conveyor belt in order to clean up excess bark with a rake. He lost his balance and fell onto the bark accumulation belt's crossbar, suffering five broken ribs. He was hospitalized.

An employee was resetting the chain at the core station. He stepped onto the foot pedal that raises and lowers the hoist as he grabbed the chain and sustained amputation of his left index and ring fingers.

The employee used their right hand to try and free their left and the right hand also became caught in the machine.
