An employee was stacking pallets in a trailer and stepped on a broken pallet puncturing his right foot.

An employee was riding on a powered pallet jack near a sewer drain. The pallet jack hit a dip near the sewer drain, and the employee s left foot hit a clamp truck parked in a charging station. He suffered a cut to the big toe, as well as fractures in the toe and the foot.

An employee went to step off material handling equipment to grab their jacket when the throttle of the vehicle engaged. The vehicle reversed and the employee's left foot was crushed between two material handlers, resulting in a fracture.

An employee was working at a height of approximately 20 feet and was standing outside his forklift's basket attachment. His forklift was struck by another forklift, causing the employee to fall to the warehouse floor below. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.
