An employee was backing an electric industrial carrier into a dolly when their fingers were caught between the vehicle and the bar on the line resulting in amputation of the fingertips on their right middle and ring fingers.

A forklift struck an overhead fan and some racking. The fan fell onto an employee working on the dock, who suffered lacerations and ruptures to the spleen and liver. The employee was hospitalized.

The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
