On 11/29/2018 at approximately 7:45 a.m., an employee was positioning a choker strap between a pipe and a reducing "T" as it was being lifted when the pipe fell/slid off a skid and pinched/crushed the employee's right thumb between the choker strap and the pipe.
An employee was performing abrasive blasting when a pipe made contact with his blasting hood and slid down his chest and waist area, knocking him to the ground. He suffered fractures on his spine and pelvic area.
