Employees were working to secure a low-hanging feeder. An employee was elevated in a bucket at a height of approximately 12 feet and was pulling the lashing wire. The lashing wire flipped up and struck the lower phase (120 volts) of power that was 2.4 feet above the feeder and strand. The employee sustained electrical burns to their chest, both hands, and the right forearm.

An employee was walking in a parking lot when he tripped over a bolt protruding from the ground and fell. He suffered two fractures in his right elbow, as well as a torn tendon leading to the elbow. He was hospitalized and required surgery.

An employee was on an extension ladder, installing cable from a pole in an alley. The employee fell from the ladder to the ground and suffered a broken left arm.

An employee was performing an inspection of a bridge crane while working from a 19-foot scissor lift. The employee was going to use a pendant to move the crane to access a different portion of the crane for visual inspection. The scissor lift guard rails were below the girder of the crane to avoid contact. The tow arm for the crane collectors contacted the guardrail of the scissor lift and caused it to tip over. The employee stayed inside the scissor lift basket as he fell to the floor. The employee sustained fractures to the right arm and hip.

An employee was on a forklift that was elevated approximately 20 feet and was auditing inventory. The forklift malfunctioned and the employee fell down to the surface below, causing them to sustain multiple fractures.

An employee was approximately 5 feet up on a ladder while drilling a hole for cable installation. A wasp attacked him. As he was coming down the ladder, he landed flat on his feet on the ground, and his right leg was twisted. The employee sustained a fractured tibia.

An employee had been installing an outside cable line at a residence. The employee became nauseous from working in the heat. The employee was hospitalized with heat exhaustion and dehydration.

An employee tripped at an elevation change while approaching a home. He fell to the ground and hit his head on a rock, suffering a contusion to the left eyebrow and a possible concussion.

An employee climbed to the top of a ladder at a customer's house. The ladder shifted and the employee fell 28 feet to the ground. The employee was hospitalized for a lung contusion and a right foot contusion.
