An employee was walking toward an entrance door. He tripped on the edge of the walkway and fell, suffering a broken left kneecap and right wrist.

An employee was pushing logs into the pasteurizer bottom deck with their left hand, while their right hand was on the track on a plastic sliding door. As the employee was finishing up the task, the plastic sliding door moved, scraping the right little finger that was in the track and resulting in a fingertip amputation.

An employee was operating and standing on a dock plate when he slipped. While trying to catch himself, his right middle and ring fingers were pinched in between the plate's joint and lip, causing an avulsion to the soft tissue on the fingers.

A conveyor was stopped, but then became unstuck and started moving. An employee's right hand was pinched between the conveyor and a support bracket on the side. A quarter-inch of the employee's index finger was amputated.

An employee was making a delivery to a restaurant when an unknown insect bit or stung the employee. The employee developed an infection and was hospitalized.

An employee was stepping out of a car when they slipped and fell on ice, resulting in a femur fracture.

An employee was working in the forest flagging an area to be harvested. He took a step and his foot slipped on a stick hidden under the leaves, causing him to fall to the ground. He landed on his right foot/lower leg resulting in a fractured tibia.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

An employee was adjusting a conveyor belt with a tool. The belt broke, causing the tool to fly back and lacerate the employee's head.

An employee was carrying a bucket of acid to his workstation for cleaning purposes. He slipped and fell, and the acid spilled into his eyes. He suffered eye injuries and was hospitalized.

Two maintenance workers were troubleshooting an actuator. They removed the end covers to check for air leakage around the piston head. When they connected an air hose to the side of the actuator and applied pressure, the piston head blew out of its housing. It struck one of the employees in the chest, causing the employee to fall to the ground. The employee suffered an abdomen contusion, resulting in a laceration to the liver, and was hospitalized.

On March 25, 2025, an employee was closing the hurricane-rated sliding door and pinched her right index finger between the door and the doorframe, resulting in a fingertip amputation.

An employee was troubleshooting a machine when he slipped and went to brace himself. His hand entered the machine and the blade contacted his finger, resulting in a fingertip amputation.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
