While stepping from the floor of the elevator (which was raised 4 feet off the floor on sawhorses) to an 8-foot fiberglass ladder, he missed his rung and fell backwards, landing on his shoulder on the floor. He suffered a shoulder injury.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

SRM Concrete

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

U.S. Postal Service - Los Angeles P&DC

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee walked into the chairlift hoist way and the chairlift platform was not there. The employee fell 10-12 feet to the chairlift platform on the first floor. The employee sustained a fractured femur.

An employee was dumping trash into a compactor when he lost his balance and fell approximately 5 feet into the trash compactor. The compactor was not in use at the time. The employee was hospitalized with multiple fractures.

Employees were installing components of an elevator from inside the elevator and removed temporary hoisting equipment when the elevator descended from the fourth floor to the pit floor. One employee was hospitalized with fractures to their fibula and a rib, and the other employee was hospitalized with fractures to both feet.

An employee was standing on the edge of the dumpster while pulling out cribbage. The employee fell to the ground, sustaining a fractured elbow and hip. The employee was hospitalized.

An employee was using a magnetic drill to drill a hole on an elevator door. His left glove became entangled with the drill bit, which partially amputated his left thumb below the last knuckle and removed several inches of a tendon. The employee's left middle finger was also severely lacerated. He was hospitalized and received stitches.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

Roby's Propane Gas

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
