An employee slipped and fell in a restroom, suffering an injury to the left ankle that required surgery.
On or about August 31, 2016, an employee working in KC-3 maneuvered a floor tile. The floor tile slipped and contacted the employees fingertip between the edge of the tile and the floor opening. The employee suffered a partial amputation of the right middle finger.
