On or about August 31, 2016, an employee working in KC-3 maneuvered a floor tile. The floor tile slipped and contacted the employees fingertip between the edge of the tile and the floor opening. The employee suffered a partial amputation of the right middle finger.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

An employee was manually moving a tall cart filled with pints of ice cream within the facility. While transferring the cart toward the exterior of the receiving area, the cart passed over a floor drainage grate, became unstable, and fell toward the employee, pinning him underneath. An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.
