On November 9, 2023, an employee lifted a 70-pound package of paper bags, put it in a cart, and then pushed the cart to the front of the store by the cash registers. The employee sustained a collapsed lung.

An employee was providing medical treatment to injured persons involved in a motor vehicle accident. The employee was walking up and down a low-grade embankment carrying injured persons on transport equipment such as backboards. The employee sustained chest pain, and a possible cardiac related event with an elevated heart rate and elevated blood pressure.

An employee was moving merchandise in boxes, rolling containers, and totes, from a truck to store shelves when they sustained back and neck injuries.

An employee was using a shovel to dig a hole for a pipe when he suffered a broken vertebra. He was hospitalized.

On June 16, 2023, an employee was elevated in a scissor lift and was removing a pipe from a structure when they began experiencing back pain. The employee sustained a ruptured abdominal aneurysm from lifting/carrying/pulling pipe.

An employee was manually moving a tall cart filled with pints of ice cream within the facility. While transferring the cart toward the exterior of the receiving area, the cart passed over a floor drainage grate, became unstable, and fell toward the employee, pinning him underneath. The cart and its contents landed on his lower right leg, resulting in multiple fractures.

An employee was on a stage hosting a customer event. While conducting a raffle drawing, the employee stepped backward off of the stage, lost their balance, and fell partway to the ground, bracing themself against the stage wall and a speaker. The employee sustained a compound fracture to their leg that required surgery.

An employee was manually picking up the lid of an etching machine. The lid slipped and pinched the employee's middle fingertip resulting in amputation of flesh from the fingertip.

On March 23, 2021, at approximately 9:15 AM, an employee was positioning a steel frame for shipping and pinched the tip of his left ring finger between two metal pieces when he set it down. The employee's left ring fingertip was amputated at the nail bed.

While leaving a building, an employee fell down a stairway and suffered injuries to the left and right ankles.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
