An employee was working outside when they began to experience heat-related symptoms.

An employee's hand slipped while changing a belt on an air handling unit. The employee's left pinky, ring, and middle fingers were lacerated and fractured as they were caught between the belt and the pulley. The unit was powered off at the time of the incident and a lock out/tag out procedure was followed prior to starting work on the unit.

An employee suffered a fractured right fibula while uninstalling recording equipment from a military vehicle.

An employee was helping a forklift driver position the forks behind a vehicle to be picked up. The forks were raised, and the employee's right thumb was crushed between them and the vehicle.

An employee completed a shift during which she moved patient beds and stretchers. While at home, the employee experienced back pain and went to the hospital, where she was diagnosed with spinal fractures.

An employee was lifting and carrying a heavy box. After putting the box down, the employee had back pain and weakness in their left leg. They were hospitalized, having suffered a back sprain.

An employee sustained a lower back strain while assisting a client who was about to fall.

An employee was turning over a patient and felt a pull in her abdomen. The employee sustained a hernia.

An employee was working on the wharf recouping lumber boards (2x4s and 2x6s that can vary from 8' to 16' in length) from bundles that had become misplaced when they sustained a right rotator cuff tear requiring hospitalization.

An employee's hand was caught in a conveyor belt as they worked to clear a jam in a piece of equipment. The employee suffered a dislocation to the right elbow, as well as a broken ulna near the elbow, and was hospitalized.

An employee was changing AC filters while on a ladder. The employee stepped up to the second-to-last rung when they slipped and fell about 5 feet to the ground. The employee sustained fractures to their scapula and ribs, a collapsed lung, and a back injury.

An employee was performing restoration activities outside a 45-gallon condensation tank of a boiler system. As they were performing post-maintenance activities after repairing one of the two discharge pumps, one of the discharge valves would not open. The employee closed the intake pump to cool off the system and open the discharge valve. Two employees opened the inlet valve to return the tank to service. After they cracked open the inlet valve, a side wall of the tank blew out, causing the injured employee to suffer steam burns to half their body.

On August 19, 2025, an employee became ill and dehydrated after working outdoors for eight hours, participating in training activities. He was hospitalized.

An employee was ascending a fixed ladder and struck his head on an overhead object that was approximately 20 feet above ground level. The employee sustained neck injuries and tingling in his fingers. The employee was hospitalized.

An employee was drilling a hole into the sealed hold of a vessel so a marine chemist could test its atmosphere. Upon drilling the hole, a gas that was under pressure was released and caught fire at the drill motor, where the employee was holding it. The employee was burned.

An employee was unlocking containers on deck. The employee was de-lashing while standing on the edge of the lid. The lashing bar he was holding came off the cone, causing him to turn 90 degrees and strike a railing behind him. The employee sustained four fractured ribs.

An employee parked a side-by-side on the fixed internal ramp aboard a cargo ship. They exited the side-by-side and went to inspect the cargo below. The side-by-side rolled toward the employee and struck them as they were walking toward the edge of the ramp. The employee was knocked off the ramp and landed on the lower deck approximately 8 feet below. The employee was hospitalized with a fractured shoulder.
