Final narrative
While clearing a jam in the head rig chipper an employee had to remove the chipper's lid. When lifting the lid, it was found that a pin which allows the lid to pivot, was not in place. This caused the bottom of the lid to rotate and roll over the bump stop. The lid then rolled over towards the employee, striking the employee in the left arm and the front of his hard hat. The employee sustained a broken left humerus.

On October 26, 2023, at 1:00 AM, an employee was unjamming a locked-out belt on a machine that sets up wooden boards. When the wood came out, it impacted his right hand, knocking it into a guard. The guard of the machine then amputated the middle finger at the middle knuckle.
An employee was moving 20-hp electric motor weighing approximately 280 lbs. The employee was sliding the motor on the floor in the storage area using their foot and guiding it with their hands. The employee's finger was pinched between the shaft of the motor they were moving and a stationary motor resulting in an amputation of the left pinky finger.
