An employee was moving 20-hp electric motor weighing approximately 280 lbs. The employee was sliding the motor on the floor in the storage area using their foot and guiding it with their hands. The employee's finger was pinched between the shaft of the motor they were moving and a stationary motor resulting in an amputation of the left pinky finger.

On October 26, 2023, at 1:00 AM, an employee was unjamming a locked-out belt on a machine that sets up wooden boards. When the wood came out, it impacted his right hand, knocking it into a guard. The guard of the machine then amputated the middle finger at the middle knuckle.

POTLATCHDELTIC

While clearing a jam in the head rig chipper an employee had to remove the chipper's lid. When lifting the lid, it was found that a pin which allows the lid to pivot, was not in place. This caused the bottom of the lid to rotate and roll over the bump stop. The lid then rolled over towards the employee, striking the employee in the left arm and the front of his hard hat. The employee sustained a broken left humerus.

An employee was conducting maintenance on a cut-up system (merchandiser) when the employee walked by a photo-eye and triggered the log kicker. The log kicker rolled a log into the employee's lower leg and broke it.

TForce Freight

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

FORD MOTOR COMPANY

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

UFP Structural Packaging, LLC

An employee was operating a panel saw when a piece of wood got stuck in the dust collection vent. He was clearing the jam when the pressure bar (that holds the material) came down on his right hand and crushed his fingers. He sustained partial amputations to the middle and ring fingertips.

MacDonald & Owen Lumber Company

On September 3, 2025, a lumber handler was cutting a board with a circular saw when the saw blade grabbed his glove and pulled his hand into the saw. The blade amputated his left index finger and lacerated his left ring finger.

Menard, Inc.

An employee was adjusting self-dumping steel hopper to get it to latch onto the tines of a forklift. He rocked the hopper back and forth to dump. The hopper slid off the forklift and fell about 2-3 feet down onto his legs. The employee sustained a cut on his head, a bruise on his face, and both his ankles were fractured.

Sun Mountain Lumber

An employee was using his left arm to pull a piece of lumber from a chain, which was on his right side, to stack it on a lumber bunk positioned to his left. His left middle fingertip was crushed between the end of the lumber he was moving and the stationary lumber stack behind him, resulting in an amputation.

NATIONAL MILL MAINTENANCE, LLC

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

Johnson Industrial Coatings, LLC.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
