An employee was working at a carcass splitter when a carcass became caught on the splitter blade. As the employee attempted to clear the carcass, the line restarted and the blade amputated the employee's left index finger to the distal interphalangeal joint. The blade was unguarded at the time of the incident.

While driving a powered industrial truck, an employee's foot slipped off the side and his calf was pinned between the truck and the wall. He suffered tissue damage in his calf.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

A warehouse employee was operating a forklift in the freezer area. While making a turn, the forklift slid on condensation that had accumulated on the freezer floor. The forklift struck a yellow pole that designates the exit path and the employee s left heel became caught between the forklift and the yellow pole, resulting in a heel pad avulsion, or rupture of the heel fat pad. The employee's injury required surgery and hospitalization.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was working in the food service warehouse when he experienced chest pain, difficulty breathing, and lost consciousness. The employee sustained carbon monoxide poisoning.
