An employee was attempting to retrieve an object off of the ground. He leaned over and braced himself with his left hand using part of a forklift. His hand contacted a metal fan inside of the forklift, resulting in a left index finger amputation to the first knuckle.

An employee was unstrapping a load of pipe on a trailer for delivery at a customer's jobsite. A pipe rolled and struck his head, causing a head injury.
An employee was transferring 8-inch pipe joint restraints from one pallet to another. The restraints were dropped, causing the amputation of the employee's left ring fingertip.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.
On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
