An employee was working at a pug mill, loading clay chunks into a wad mill and offloading pugs that came off the wad mill. An extrusion plate, which pushed chunks of clay off a push cart, pinned the employee's left leg against the push cart, causing injuries and fractures to the leg.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was carrying a bucket of water to the work bench when he tripped. His right foot became stuck between the base of the 28" mixer re-screener (machine was not in operation) and anchor bolts protruding from the floor. The employee fell with his foot stuck, resulting in a fractured ankle that required surgery and hospitalization.

An employee was on a step ladder, loosing a bolt connected to a machine. He fell off the ladder, landed on his back less than 4 feet below, and hit his head. The employee sustained head trauma and was hospitalized.

An employee was using an industrial table saw when it amputated his thumb and caused a severe laceration to his palm.

An employee was operating a press when it caught his fingers. He suffered partial amputations to the index finger and middle finger.

An employee was using a punch press to punch a magnetic strip when the material jammed. When the employee attempted to unjam the press, their left index, middle and ring fingers were caught in the press. The employee sustained an amputation to the tip of their left middle finger and received stitches to their left middle, ring, and index fingers.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
