PARK RANGE CONSTRUCTION, INC.

A crew had completed drilling a 4.5' diameter shaft for a micro-pile installation and were placing thread bar reinforcement into the drilled shaft. Two employees used pipe wrenches to hold the bar and lower it into the shaft. The first employee was holding his wrench approximately 6' above the ground and the second employee was holding his at waist height when he lost his grip on the wrench. The full weight of the bar fell on the injured employee's wrench, which pulled his wrench and hand down to the ground. His right middle fingertip was pinched between the bottom of his pipe wrench and the ground, resulting in a partial amputation to the distal phalanx.

An employee was checking the function of newly installed actuators when they were struck by a pressurized release pipe. The employee suffered fractures to the orbital socket and shoulder, requiring hospitalization.

On October 7, 2025, employees were conducting trenching operations for the installation of a new sewer line system. The excavator's bucket was detached and put inside the trench as it was being exchanged for a different-sized bucket. An employee entered the trench with a shovel to move some dirt located at the front end of the pipe. The excavator bucket shifted and slid, striking the employee s right foot and trapping it against the ground. The employee sustained a fractured right tibia.

WPM-Southern, LLC

Employees were working to dislodge a large truck that was stuck in a rut using tow straps and a front-end loader with the bucket removed. An employee connected the straps to the loader and truck and another employee slowly engaged the loader to close the coupler. The injured employee's arm became caught in a pinch point. The employee was hospitalized with a fractured arm.

Garrison Enterprise Inc.

An employee was lifting a piece of 30-inch line stopping equipment using a jack screw. A lifting eye weld snapped and the load dropped. The end of the jack screw then landed on the employee's heel and crushed it. The employee was hospitalized.

ABC Supply Company

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
