An employee was adjusting a guard on a box sealing machine when they slipped on tea oil, causing their right thumb to get stuck in the chain/sprocket. The distal portion of the employee's right thumb was amputated.

The employee fell onto the porch roof, then to the ground below, a distance of about 10 feet. He suffered broken ribs, lacerations and facial abrasions, and a potential facial fracture.

An employee was going door-to-door selling pest control services during 80+ degree weather. The employee sustained heat stroke and collapsed.
