An employee was troubleshooting a paving machine. He was cleaning the grease on the sprocket using a rag wrapped around his finger. The sprocket was running at the time and caught his hand and amputated the left middle finger.

The injured employee was collecting 2x4s to build concrete forms on the bridge bent above. A 4x6-foot section of wood dropped from above and struck the injured employee. They sustained multiple spinal fractures in the upper and lower back and fractured ribs.

An employee descended the stairwell of the flatbed of a traffic control truck and shut the stairwell/platform door behind him. The door shut on his left third finger, resulting in amputation of the fingertip.

An employee was helping to set concrete traffic barriers by guiding a barrier clamp weighing approximately 600 pounds that was connected to an excavator. The employee guided the clamp down on top of the barrier successfully, which then disengaged the locking pin and allowed the clamp to scissor and grab the barrier to raise it again. As the excavator operator hoisted the barrier clamp to move the segment of barrier, the employee's left ring fingertip got caught in the scissor area of the barrier clamp, resulting in a transphalangeal fingertip amputation.

An employee was using an auger to drill holes for surveying operations when his hand and arm became entangled in the auger. The employee sustained the amputation of his left thumb and his middle finger to the second joint as well as fractures to the hand and arm.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
