On January 7, 2019, a maintenance employee was replacing a fork on a forklift when the fork smashed his finger between the fork and the forklift. He was hospitalized with a broken right middle finger.

An employee was on a double-end rider in an aisle selecting product when his left leg was pinned between the rack and rider.

An employee was using a forklift to load a tractor trailer when the forklift skidded into a wall. The employee sustained an arm fracture.

An employee was training on an rolling container carrier on the dry, non-frozen storage side of the distribution center. She made a turn around the end cap and her right foot struck the rail protecting the endcap racking. The employee's foot was fractured.

An employee was a passenger in a utility vehicle that was driving up a hill on a wooded trail. As the vehicle made a right turn, three fingers on the employee's right hand were caught against a tree and fractured.

An employee was operating a forklift when his left leg was caught between the forklift and a bollard resulting in a broken leg.

An employee was making repairs to a milling machine located in the mold shop. While using a screwdriver to make adjustments, the machine energized and struck the employee. The employee's right arm was fractured.

An employee was breaking down dunnage to prepare totes and trays to go through a tote washer. A tow motor driver asked the employee to move a cart that was in the way so the tow motor could remove a gaylord. The employee moved the cart and then stepped away from the tow motor. As the employee moved to an area between carts, his right foot got caught by a cart, causing him to twist and fall on top of a cart, landing on the left flank of his torso. The employee suffered a hemorrhage on his left flank.

An employee tripped over a computer cord and fell fracturing their femur.

Three employees were installing a transfer rail in a press. There were two employees holding the rail and the injured employee was at the end of the rail guiding the rail into the bracket. The task was done manually with no mechanical equipment. The employee had their finger at the end of the rail and went to make sure the rail was lined up while the other two employees pushed it into the bracket, causing their finger to become pinched between the rail and bracket. The employee's middle fingertip was amputated. Protective gloves were worn at the time of the incident.

An employee was walking across the dock, interacting with forklift drivers on what was being loaded. A forklift drove forward with a tall load on the forks. The employee was struck by the load (running board/rocker panel rack) on the forklift. The employee sustained several contusions and an injury to their right Achilles tendon that required surgery.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
