An employee was using an air jack and a can press machine during a die change. The employee's left index finger was caught between the air jack and the machine, causing a fingertip amputation.
On June 9, 2022, an employee was installing a timing belt on a glue/banding machine. The employee's right ring finger entered a nip point between the belt and the machine resulting in a fingertip amputation.
On Saturday 10/10/15 at approximately 5:00 AM, an employee's hand slid underneath a fixed guard at the Department 5540 Pleater. The task was done manually with no mechanical equipment. The employee had their finger at the end of the rail and went to make sure the rail was lined up while the other two employees pushed it into the bracket, causing their finger to become pinched between the rail and bracket. The employee's middle fingertip was amputated. Protective gloves were worn at the time of the incident.
