An employee was using an oxygen lance to add oxygen to a furnace to increase the temperature. An explosion occurred, causing shrapnel to strike the employee in the right collar bone, resulting in a fracture and open wound. The employee was hospitalized.

An employee was inserting a metal wedge between a tundish cart and a stopper to prevent movement of the cart. The wedge twisted and the employee's fingers got caught between the cart and the stopper, resulting in a partial amputation of the left ring finger.
