An employee was grinding glue off wheels on top of a corrugator machine. The employee's right index finger was caught between two of the machine's rollers, causing the amputation of the fingertip.

The injured employee was cleaning parts inside a machine that makes boxes. She went to grab a piece of material when while the chain and sprocket was being manually moved and her left index finger was caught in the mechanism. The employee's fingertip was amputated with bone loss. The machine was locked out/tagged out at the time.
An employee was turning off a water valve on a roof using a wrench. A pipe was knocked loose and, in turn, knocked the employee off the roof. The employee landed on the ground below, suffering a broken ankle and a spleen injury.
