An employee was turning off a water valve on a roof using a wrench. A pipe was knocked loose and, in turn, knocked the employee off the roof. The employee landed on the ground below, suffering a broken ankle and a spleen injury.

The injured employee was cleaning parts inside a machine that makes boxes. She went to grab a piece of material when while the chain and sprocket was being manually moved and her left index finger was caught in the mechanism. The employee's fingertip was amputated with bone loss. The machine was locked out/tagged out at the time.

An employee was putting out a fire in the baler chute by spraying it with a fire extinguisher. Smoke and the fire extinguishing agent blew back into their face. The employee was hospitalized due to smoke/extinguishing agent inhalation.

On July 10, 2023, an employee was crossing a conveyor line (11 3/4 inches high) at the crossovers/catwalks. As he stepped up onto the conveyor with his right foot and planted himself, he fell. His left knee hit the ground, while his chest landed on the conveyor line frame. He suffered seven broken ribs, four on the right side and three on the left side, below the sternum.

An employee was using a hoist to lift a bundle while standing on an elevated platform. The employee fell about 8-10 feet to the ground, breaking three ribs and a bone in the back. The employee was hospitalized.

An employee was troubleshooting the control setup for a variable air volume box above a ceiling. The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder. His left hand passed across the ceiling grid and was lacerated. He also suffered an electrical burn to the right hand and was hospitalized.

An employee was working as a fire watch. The employee was exposed to anhydrous ammonia and was not able to see while climbing down a ladder due to eye irritation. The employee fell and was hospitalized for a broken ankle.

An employee was working from a ladder and adjusting a spreader guide roller with a T-handle Allen wrench. The employee came into contact with an electrical junction box/wires, was shocked, and fell off the ladder to the concrete floor. He suffered a broken right-side rib, a puncture in the right lung, and an electrical shock burn to the right palm.

An employee was on a 10-foot ladder, working to insulate 24-inch piping that was 12 feet above the ground. The pipe jack/steel tube used to raise the pipe slipped, causing the steel tube to strike the employee's face. The impact caused the employee to fall off the ladder and sustain fractures to their left tibia and fibula.

An employee was on a rooftop, supervising the lifting of a 30' x 30' structure manufactured from I-beams. The employee was knocked to the ground and the suspended load settled on them. The employee sustained a head injury, loss of an eye, and fractured ribs.

On October 22, 2025, an employee was working to put a conveyor chain back on the track of a conveyor when their fingers were caught between the chain and the chain track, pulling their right hand into the sprocket. Four fingers were amputated.

An employee was entering a building and reached out to catch a closing door. The door closed on the employee's finger, resulting in the partial amputation of the fingertip and nailbed.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
