Final narrative
On February 19, 2019, while sandblasting outside a tank, an employee tripped over the sandblaster hose and fell to the floor, suffering a broken left femur.
An employee was using a tape measurer to measure the circumference of a steel tank that was sitting on rollers when slack in the tape measurer began to create a loop that was getting hung up. As the employee reached for the loop, he inadvertently activated the roller. His left pinky was then caught and smashed between the roller and the tank, resulting in a surgical amputation.
An employee was cleaning a paint gun using MEK (methyl ethyl ketone), when the high-pressure stream of MEK was injected into his left palm, requiring hospitalization.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was standing on a 10-inch high working platform (36.75"x24"x10") verifying that tanks were being picked up properly by an overhead conveyor.
