An employee inserted an Allen key into the soft step adjuster of a lip groover to adjust the machine's tension. His left pinky finger was positioned near the machine shaft and shaft housing. The work cell was not de-energized at the time. As he was making the adjustment, the machine cycled, pulling the shaft into the housing and amputating approximately half of his left pinky finger.

An employee was walking back from retrieving a piece of equipment (a steel hook) to attach to a vessel when he tripped on the edge of a mat on the floor. The fall caused a laceration to the employee's elbow; he was hospitalized.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.

An employee tripped over a concrete curb stop in a parking lot. The employee fell to the ground, landing on their left hip and breaking it.

An employee was standing on a 10-inch high working platform (36.75"x24"x10") verifying that tanks were being picked up properly by an overhead conveyor. The employee turned and was stepping off the platform when their left work boot caught the edge of the platform. The employee fell to the concrete floor and landed on their right side. The employee's right hip was fractured.

An employee was working on a vessel, attaching the base plate ring to the legs, when the base plate shifted about 3 inches. The employee's right foot became pinned between the base plate and the floor. Three toes were crushed. The employee was hospitalized and required surgery, resulting in partial amputation of their toe tips.

Two employees were constructing a 30-foot-tall aboveground storage tank, fitting a double roof on the tank. Half of the roof had been stitch-welded to the tank, and the employees were fitting/tacking down the second half of the roof, when the roof shifted and fell in. One employee fell and sustained fractures to their right foot and shin. The second employee suffered a broken back. Both employees required hospitalization.

Two employees engaged in a verbal altercation. The injured employee was punched in the face and fell, hitting their head on the concrete floor. The employee sustained a brain bleed and blunt force trauma to the face.

An employee was unloading propane tanks from his truck using the crane attached to the truck when a propane tank struck him and he fell to the ground, fracturing his right ankle.

An employee was talking to a child on the playground, when the child grabbed her hand to lead her away. The employee stepped, lost her balance, and fell over. The employee was hospitalized with a fracture of her lower left leg and required surgery.

An employee was struck by a plasma cutter and suffered a broken left tibia and fibula. The employee was hospitalized.

An employee tripped on a rotor on the floor of a store, fell, and suffered a broken right thumb.

An employee was walking through a parking garage. She tripped over a curb and fell forward onto her right knee, then her left knee, both hands, and her face. She suffered an injury to the left knee that required surgery, as well as scrapes on the chin and both palms and soreness in the left wrist.

An employee was using a hose to clear storm drains after a sewer backup. The employee fell backward and his head struck the curb, resulting in an Injury on the right side of his head, generalized bruising, and pain in his left ankle, right hip, left shoulder, and neck. The employee was hospitalized.
