Final narrative
An employee was operating a bucket truck picking up road signs along the side of the road at the end of the day. The employee was on the back of the truck strapping down the signs. When he stood up, he struck his head on the bottom of the bucket, lost his balance, and fell approximately four feet off the side of the truck to the ground breaking the lower left leg.

An employee was working outdoors on a highway construction project when he began feeling hot, nauseated, and had leg cramps due to excessive heat. The employee was hospitalized for heat illness.
An employee was performing road construction in a closed lane of an overpass. As he was assisting a co-worker in saw-cutting a joint in the overpass, he was struck by an automobile, resulting in a broken right shoulder, right leg, and left middle finger, as well as multiple scrapes and lacerations.
