An employee was caught between a cart and a powered industrial vehicle while bending down. The vehicle's handle pinned the employee's left arm against the cart, breaking the arm and wrist.

An employee parked and dismounted her tugger to connect a line of empty carts to the vehicle. Another tugger entered the area towing seven full carts. As the tugger began turning right, the last cart struck the final cart of the line the employee was connecting. Upon being struck, the line of empty carts pushed forward and the first cart in the line struck the employee's left leg, resulting in a tibial plateau fracture.
After moving empty containers with a forklift, an employee began to travel forward in the forklift. While re-entering the main aisle, the left fork struck the roll cage of an oncoming forklift that was travelling in reverse. This caused the injured employee's forklift to tip over onto a guardrail. The upper rail of the guardrail entered the top of the forklift's roll cage, pinning the employee's legs against the seat. The employee sustained a fractured leg.
