Final narrative
An employee was on a knee cart. Its handle grip came loose as she turned and she fell to the tiled floor, breaking her lower right tibia and fibula.
An employee was assessing a customer's roof when the employee's ladder slid away, causing the employee to fall approximately 4 feet. The employee suffered four broken ribs and a scalp laceration.
On November 11, 2015 at approximately 12:15 pm smoke from an electrical fire in the building activated the fire alarm. An announcement was made over the PA system that it was a false alarm, so employee(s) remained at their work stations. Moments later the employee(s) were told to exit the building. The fire department was called and arrived on scene (for approximately 30 minutes). The employee coughed for the remainder of the day and the rest of the night. Due to difficulty breathing and excessive coughing the employee sought care at the emergency room. She was admitted.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
