An employee was inspecting the attic of a home when she fell through the garage ceiling to the concrete floor below, breaking her femur.

An employee was assessing a customer's roof when the employee's ladder slid away, causing the employee to fall approximately 4 feet. The employee suffered four broken ribs and a scalp laceration.

On November 11, 2015 at approximately 12:15 pm smoke from an electrical fire in the building activated the fire alarm. An announcement was made over the PA system that it was a false alarm, so employee(s) remained at their work stations. Moments later the employee(s) were told to exit the building. The fire department was called and arrived on scene (for approximately 30 minutes). The employee coughed for the remainder of the day and the rest of the night. Due to difficulty breathing and excessive coughing the employee sought care at the emergency room. She was admitted.

An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

On December 27, 2023, an employee was delivering mail to an apartment complex when their knee gave out and they fell to the ground. The employee was hospitalized with a left tibia fracture.

An employee was walking to the inventory cage when her ankle gave out and she fell to the floor. The employee sustained a femur fracture and was hospitalized.

An employee was dislodging a pallet that was stuck when boxes of product fell onto them, causing them to fall to the ground. The employee suffered a fractured femur.

An employee was exiting an elevator when he lost balance and fell to the floor, resulting in a fractured hip.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

On October 20, 2025, an employee was installing a security system. He was on a 6-foot A-frame ladder to drill holes in a wall when he fell from the ladder and struck his head on the floor. The employee was hospitalized with a fractured skull.

An employee was playing kickball as part of a team building activity when they fell and suffered a dislocated left foot.

An employee was replacing a light fixture in an office ceiling and sustained a possible electric shock. The employee fell from the stepladder and suffered fractures to the face and left shoulder.

An employee and a coworker were handling a cutter blade when the blade cut his thumb, resulting in a possible amputation.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
