An employee became overheated while working at a plywood production facility. The employee sustained dehydration, heat syncope, rhabdomyolysis, and an acute kidney injury.

An employee was removing a tube from a dryer when the tube partially came out. As he pushed the tube toward the door opening, the tube came free and the employee's momentum carried him forward. His right little finger was pinched between the door frame and the dryer tube, resulting in an open fracture. The employee was hospitalized and had surgery.

An employee slipped on ice in a parking lot and fell, breaking two vertebrae in the lower back.

An employee was troubleshooting a conveyor when his right hand was caught between the conveyor frame and the crush roll. His hand was broken.

An employee was assisting his co-worker to brace the products (35 inches high by 29 inches wide by 120 inches long) on the forklift. The forklift driver set the double pallet of reinforcement material on to the floor and the forklift backed out. Part of the bottom pallet on the left side broke and the materials started to wobble. The injured employee tried to stabilize it, but part of the top pallet fell on his chest causing him to fall down. As he was bracing his fall, he fractured his left wrist.

An employee was checking the function of newly installed actuators when they were struck by a pressurized release pipe. The employee suffered fractures to the orbital socket and shoulder, requiring hospitalization.

A ramp in the trunk of a delivery van struck an employee's calf. She suffered a broken leg and was hospitalized.

An employee was completing a service call and looking at a water pump when the pump cover plate struck their forehead. The employee sustained a forehead laceration and fracture requiring hospitalization.

An employee was removing chains from a steel tube. A sorting hook attachment was caught on the staging area cross member. When he raised the chains, the sorting hook snapped back and struck the employee's face causing a fractured jaw.

An employee was in the sawmill basement using a push broom and shovel to clean wood chips and sawdust. The employee's right arm was struck by a wooden board between the wrist and elbow, resulting in a fracture.

An employee was working to dislodge a piece of stray veneer from a conveyor. The employee set up a ladder against the machine and climbed the ladder. He placed his left hand on the rail of the conveyor and his fingers were caught in the chain and sprocket assembly that powers the conveyor. Three fingers were amputated. The machine was guarded at the time.

A maintenance employee was changing a shaft on a piece of equipment that had been isolated for repair. While positioning the shaft into place, the shaft fell approximately two feet and pinched the employee's left index finger. The employee suffered a partial amputation of the fingertip without bone loss.

An employee was using a cart to move saw blades. The cart tipped over, striking and pinning the employee under the blades. The employee sustained severe lacerations to their torso and legs that required surgery.

An employee was labeling and writing on the back of a slab of quartz. The material was leaned toward him when the A-frame it was resting on shifted, causing the material to fall onto his right leg and break it.

An employee performed maintenance on various machines when they began to feel hot and unwell, and they experienced cramping. The employee was hospitalized with heat stress or dehydration.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
