Final narrative
An employee was assisting his co-worker to brace the products (35 inches high by 29 inches wide by 120 inches long) on the forklift. The forklift driver set the double pallet of reinforcement material on to the floor and the forklift backed out. Part of the bottom pallet on the left side broke and the materials started to wobble. The injured employee tried to stabilize it, but part of the top pallet fell on his chest causing him to fall down. As he was bracing his fall, he fractured his left wrist.
An employee became overheated while working at a plywood production facility. The employee sustained dehydration, heat syncope, rhabdomyolysis, and an acute kidney injury.
An employee was removing a tube from a dryer when the tube partially came out. As he pushed the tube toward the door opening, the tube came free and the employee's momentum carried him forward.
