An employee was working from a raised scissor lift to fix an overhead garage door. The lift tipped over and the employee fell to the concrete floor about 18-20 feet below. The employee suffered a head injury, broken ribs, and a broken pelvis.

An employee was removing aerial service wires from an extension ladder when he fell 14 feet from the ladder. The employee sustained fractures to his pelvis, right hip, and ribs.

An employee was working from an extension ladder that was resting on a telecommunications line running between two wooden poles. One of the poles broke, causing the line to drop. The employee fell with the ladder, landing on the asphalt pavement and was hospitalized for a head injury.

An employee was moving/relocating a desk and an ergonomic chair when he slipped and fell to ground, hitting and injuring one of his knees.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was descending a ladder that was on a strand when the ladder shifted, causing the employee to jump off and fall about 5.5 feet to the ground. The employee sustained a fractured right ankle.

An employee was cutting boxes using a box cutting tool and lacerated an artery in their wrist. The employee was hospitalized.

An employee was removing 2,500-foot reels of 1 cable-in-conduit (CIC) weighing ~1,022 lbs. from the back of an enclosed delivery truck. The employee was manually rolling a reel off the truck when another reel shifted and rolled forward, crushing his right hand between the two reels. The employee's right ring finger was partially amputated without loss of bone.

An employee was on an extension ladder working to replace a service line to a customer's home. The existing service line broke, causing him to fall off the ladder. He was hospitalized with a dislocated elbow and fractures in his left wrist requiring surgery.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
