An employee was removing 2,500-foot reels of 1 cable-in-conduit (CIC) weighing ~1,022 lbs. from the back of an enclosed delivery truck. The employee was manually rolling a reel off the truck when another reel shifted and rolled forward, crushing his right hand between the two reels. The employee's right ring finger was partially amputated without loss of bone.

An employee was descending a ladder that was on a strand when the ladder shifted, causing the employee to jump off and fall about 5.5 feet to the ground. The employee sustained a fractured right ankle.

Greenbrier Rail Services

An employee was using his left hand to push an axle down the line on a raised track work area while holding the remaining axle back with his right hand. A finger on his right hand was caught between the rail and the dust guard of the axle, resulting in a fracture and a laceration.

An employee was moving metal out of a cart. He had removed one piece and was working to move another when the metal shifted. His right ring finger was crushed between the two pieces, and he suffered a partial amputation to the finger pad at the tip.

A load of steel angles was being lowered when the lifting chain hit the load, causing it to slide toward an employee. An angle slid and pinched the employee, catching both of their legs between the beams and an angle. The employee suffered two fractured legs.

An employee used a crane to set down a bar. The bar measured 9 inches by 16 feet. The strap got stuck under the bar, and the employee asked another employee to jog the roll. The bar rolled and pinched the injured employee's left hand, resulting in a fingertip amputation.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.
