An employee pushed an unloaded pallet into a bin pallet stock/storage area (pallet flow slot return) for reprocessing, then fell to the wood floor, breaking her left hip. She was hospitalized.
An employee had just finished unloading his truck. He pulled the dock plate up and reached to pull the door down. His foot went between the dock plate and the trailer, causing him to fall to the right and break his leg. He was hospitalized, requiring surgery.
The injured employee was selecting food product from a pick slot while standing between her cart and the pick slot. Another employee approached on his tow tractor; he went to turn his tow tractor around so the hitch could attach to his cart, which was next to the injured employee's cart.
