Final narrative
An employee was assembling a roll. The shaft got stuck in the roll and as he used a press to force it out, he was struck on his left hand by flying metal debris (possibly the shaft). He left index finger was medically amputated.
An employee was operating a laser guided vehicle (LGV). The employee put the LGV in manual, got off the machine and was pinched between the LGV and conveyor, suffering a right hip contusion.
