An employee was climbing a ladder to access the roof of a building while holding insulation, lost their grip and fell 15 feet. The employee sustained a fractured left ankle.
Employees were cleaning the "skin puller machine" with a water hose. The injured employee was operating a water hose and nozzle, spray cleaning the machine. After the machine was cleaned and re-energized the employee noticed a tag of skin inside the machine and attempted to retrieve it.
