On March 27, 2019, at 8:45 PM, an employee was attempting to clear a clog when the employee's left middle finger was struck by a rotating mixer blade, resulting in a broken finger and partial amputation.

Employees were cleaning the "skin puller machine" with a water hose. The injured employee was operating a water hose and nozzle, spray cleaning the machine. After the machine was cleaned and re-energized the employee noticed a tag of skin inside the machine and attempted to retrieve it. The employee's hand contacted the rotating parts resulting in partial amputation of the employee's middle, ring and index fingers.

A temporary employee was pulling a piece of skin out of a skinner machine when his gloves were caught in the skinner blade, lacerating his right index and middle fingers and resulting in an amputation.

The employee was on top of a rail car to level the grain that was deposited into the car when they fell 13-15 feet to the ground. The employee sustained a right leg fracture.

An employee was on an extension ladder installing a satellite dish on a roof when they fell approximately 12-15 feet to the ground. The employee sustained fractures.

An employee was setting a 5-foot pry bar into a precast concrete slab and stood on the end of the pry bar to move the slab into place. The pry bar slipped out causing the employee to fall backward 15.5 feet to the dirt ground. The employee was hospitalized with a broken leg.

An employee was on a step ladder to measure and mark a crane rail for modification. The employee fell approximately 12 to 15 feet to the ground, resulting in a fractured right hip.

An employee was on a trailer securing a load of logs when they fell approximately 9-12 feet to the ground. The employee sustained brain hemorrhaging and lacerations to the thigh and above the eye. The employee was hospitalized.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.

An employee was troubleshooting a thigh deboner on a poultry processing line. As he worked to dislodge a thigh bone caught in the blade mechanism, the machine amputated the first and second digits on his left hand. He was hospitalized.

On September 25, 2025, an employee was exiting a yard truck. They slipped and fell approximately 3 feet from the top tread of the steps to the concrete below. The employee sustained a fractured left hip.

On September 17, 2025, an employee was cleaning a cutting machine when the cutting blade amputated their right index finger below the first knuckle.

Koch Foods of Mississippi, LLC

An employee was operating a stand-up forklift, picking product for an order. The forklift struck a metal rack and the employee's left arm was caught between the forklift and the rack. He was hospitalized with a fracture to the neck of his radius and an open fracture to the olecranon (elbow).

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

US Battery Manufacturing Company, Inc

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.
