A mechanic was opening a door for a production blast freezer to allow it to defrost. When the door had been secured, the employee stepped out from behind a column and was struck by a forklift, resulting in a right leg fracture.

An employee was adding oil to the extruder. The auger, which was still turning, caught and amputated the employee's thumb tip. The extruder was not locked out at the time of the incident.

An employee operating a high-reach truck ran into the forks of another high-reach truck. The forks of the other high-reach truck were approximately 1 foot above the floor. The employee sustained a fractured tibia and fibula and crushed knee on the left leg, requiring hospitalization.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was performing maintenance under a kettle. When he removed a tri-clamp on a transfer line, the hot fat and broth material in the kettle poured out onto his arm. The employee was hospitalized with burns.

An employee was walking on top of boiler to close a valve. The employee fell off the boiler and landed on the floor, sustaining fractured ribs. The employee was hospitalized.

An employee was inspecting a pipe in the rice cooking area near an oven. While checking for a clog, the fingers of their right hand were caught in an airlock at the base of the pipe. All the fingers were amputated except the thumb. The employee was hospitalized.

An employee was removing dough from the discharge at the bottom of a mixer when the scraper contacted his left index finger and amputated it.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
