RAYMOND CORPORATION

An employee was using a crane and lifting fixture. They placed a mast onto an up-ender table. While the up-ender table was raising the mast, the lifting fixture became caught on the mast and caused the mast to fall and strike the employee. The employee was hospitalized with a fractured left foot.

RAYMOND CORPORATION

On 7/31/2019, at approximately 11:07 AM, an employee was walking inside the facility when he was struck by a forklift and sustained multiple fractures to the left foot. The injured employee was hospitalized and had surgery.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

Alsco Inc.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

Equipment Depot - Romeoville

An employee was using a punch and hammer to remove a pin from the mast of a forklift. The mast was secured by an overhead crane. The mast shifted, and the employee's thumb was pinched between the mast and the punch, resulting in an amputation to the tip of his left thumb.

CROWN EQUIPMENT CORPORATION

On September 22, 2025, an employee tripped and fell over a permanent fixture (angle iron) that serves as a parking block in the back of a facility. He suffered a fractured right hip and was hospitalized.

An employee was helping a customer with repairs on their trailer. While lowering the ramp door of the trailer, the customer dropped the door and it contacted the employee's left ring finger, resulting in a fingertip amputation.

Cascade Corporation

An employee was working to lay a part in the vise of the band saw in preparation for cutting. The left vise closed, pinching his fingertips against the part. The employee's middle and ring fingertips were amputated and they sustained a laceration to the index finger requiring stitches.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.
