On March 11, 2024, an employee was removing a support for a delivery rack when a piece of metal on the support swung around and sheared the tip of the employee's right index finger, resulting in a fingertip amputation.

While moving a frame rail from left to right, an employee's finger was pinched between the frame rail bracket and a J-hook. The employee's right ring fingertip was amputated.

An employee suffered an amputation while working with a truck frame following a frame flip-over.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

On September 18, 2025, an employee was rolling a stack of two steel racks containing automobile bumpers when the top rack dropped, pinching their right little finger. The employee suffered a partial amputation.

An employee spent about 4 1/2 hours working outdoors, servicing a boom lift on a truck. He was hospitalized with heat stress.

An employee was exiting an ambulance when his foot slipped, causing him to hit either the portable aluminum step or the concrete floor. He suffered a broken left elbow.

Two employees were sliding a 200-pound steel mounting plate off a worktable onto the forks of a forklift. They slid the plate until it was half on the table and half on the forklift, and were making an adjustment to get the plate fully on the forklift when the plate pinched one employee's left ring finger. He suffered a partial amputation to the fingertip.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
