An employee was working on the motor of a stand up order picker and ran the drive to allow the brushes to break in. The employee was walking around the order picker, tripped and fell, catching his hand in the rotating metal cooling fan and sustaining a fingertip amputation.

An employee was sitting in a chair. When he stood up he lost his balance and fell to the floor. The employee was hospitalized with fractures to his left femur and hip.
