Final narrative
On April 3, 2019, an employee was unjamming a belt using a crow bar. His right thumb was smashed against an electrical panel and electrical box, causing an amputation above the first knuckle.
On February 4th at approximately 2:00 PM, a maintenance mechanic was repairing a broken chain on a debarking/log cutter machine. A compression roller (log holder) came down and pinned the employee. The employee was hospitalized with two broken vertebrae requiring surgery. The machine was not locked out at the time of the incident.
