Moving to intervene between two clients, an employee fell and broke his right ankle. He was hospitalized.

An employee was working the register when a customer attacked the employee with a knife. The employee sustained multiple stab wounds, a broken shoulder blade, and a shoulder injury.

An employee was assisting the medical team with a combative patient when the patient attacked the employee. The employee fell to the floor, hitting his head on a cabinet and the floor. The patient then tried to strangle the employee, and he lost consciousness for approximately 30 minutes. The employee sustained a concussion, traumatic subarachnoid hemorrhage, traumatic pneumocephalus, and traumatic subdural hematoma.

An employee was taking a break outside his work vehicle at a convenience store when he was attacked by a customer with a pair of scissors. He called 911. When the operator asked which way his attacker had gone, he stepped away from his vehicle and the attacker hit him with his van. The employee suffered a head injury; multiple broken ribs; abrasions all over his body from being dragged by the van; and injuries to the right shoulder, right knee, right ankle, and lower back.

An employee was assaulted by three people while helping a customer. The employee was stabbed with a box cutter and suffered a concussion, and was hospitalized.

An employee was confronting a former employee in the parking lot when a physical altercation occurred and the employee suffered a head wound.

An employee was in a one-person mobile scaffold, changing light bulbs in a church sanctuary ceiling. The scaffold fell over and the employee (who was secured in the lift basket) fell about 15 feet onto pews. He suffered broken ribs and a broken right arm; he was hospitalized, requiring surgery.

An employee was walking to meet with a pilot when they were struck by a helicopter blade, resulting in a skull fracture.

An employee was sitting with a client in a dental exam room. When she stood up at the end of the appointment, her foot got caught on a cord that operates the dental hand pieces, causing her to trip and fall to the floor on her right side. The employee was hospitalized with a fractured femur and required surgery.

An employee was on a ladder, adjusting a security camera outside a building. He fell from the ladder, landed on the ground on his back, and suffered head and back injuries that caused him to lose consciousness. He was hospitalized.

An employee was pushing a lawn mower on a slope when he slipped and his left foot went under the mower deck and contacted the blade. The employee sustained an amputation of the first toe.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
