An employee was manually lowering a lift. The elevator weight on the tensioning chain jammed between the frame and elevator; the employee's lower leg was broken as she tried to clear the jam.
A maintenance employee was adjusting the alignment of a shaft-driven conveyor belt. The rotating shaft bolt and collar caught the employee's sleeve and pulled the employee's right arm into the shaft. The employee suffered multiple fractures to the arm.
