An employee was mopping hot tar on a roof when his foot stuck to the roof. As he tried to move he fell and his arms landed in the mop bucket which contained hot tar. The employee sustained burns to the left arm and right hand requiring hospitalization.

An employee was going to install anchorage and tie off to a 6/12 pitched metal roof for a small repair job. The employee waited for the roof to dry for about an hour. However, the roof was still wet and he slipped and fell off the one-story roof onto the concrete patio of the residence. He landed on his legs, suffered a fractured knee cap, and required surgery.
