An employee was picking product from a rack 64" off the ground. The employee stepped back from the pallet on the rack and slipped off striking his leg on the order picker or the pallet and lacerating his right leg just below the knee. Fall protection was worn at the time.

An employee was driving an order picker, moving a pallet of wire to be put away. While he was backing the order picker into position to offload the pallet, his left index finger was pinched between an overhead pipe and the overhead guard. His fingertip was lacerated and part of it was amputated.
An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.
